Healthcare Trust Fund (HTF) Policy and Guidelines

The HEALTHCARE TRUST FUND (HTF) is an amount of money raised through contributions from members of this Organization, held in trust by a person or persons or deposited in a bank (whichever is practical and applicable); and withdrawn and paid out only for the purpose for which the HTF has been established;


1. PURPOSE OF THE HTF - To provide cash assistance when you get hospitalized due to illness or accident.


2. WHO IS QUALIFIED TO JOIN THE HTF?

2.1 All members duly registered as members of this Organization, provided that payments have been made for the Annual Membership Fee

( Php 100.00) , the HTF Fee (Php 50.00), and/ or

replenishments thereof as needs arise, provided further, that the HTF Agreement has been duly accomplished.


3. WHAT IS THE BENEFIT OF THE HTF?

3.1 When a qualified member will be admitted to the hospital by sickness or accident, the member shall be entitled to claim a cash assistance by the amount due from the HTF.

3.2 Such amount shall be equivalent to the HTF Fees contributed by the members who signed in for the HTF Agreement.


4. HEALTHCARE TRUST FUND AGREEMENT

4.1 Members of this Organization are encouraged to join the HTF by accomplishing the Agreement Form and Contributing the amount of Fifty Pesos ( Php in 50.00) in full.

4.2 In the event a benefit or claim has been awarded, subsequent contributions/collections will be undertaken to immediately replenish the HTF.

4.3 Any HTF member who fails or refuses to contribute the aforementioned fee especially for replenishments will cause cancellation or revocation and shall not be qualified to the monetary benefits.

4.4 If the HTF member has been cancelled or revoked and wishes to join the program again, a member can renew his/her membership in the next following year’s healthcare program.

4.5 Awarding and release of HTF shall be with supporting documents deemed necessary attached thereto.

4.6 The supporting documents are the certificate of confinement, picture of the patient, hospital tentative and / or final billing.

4.7 Failure or refusal to submit the above-mentioned documents will not be able to receive the cash assistance.

4.8 Out-patients and pregnancies are NOT included in the health care program.

4.9 The HTF member can avail the cash assistance once a year.

4.10 Should the HTF be in a bank, the amount due shall not include the initial deposit, maintaining balance, and interests that belong to the Organization.
